My interest in genital piercing goes back at least ten years to the day I tried to pierce my frenum with a rusty safety pin. Needless to say, this is not recommended piercing procedure! However it worked surprisingly well and left me with a perfectly useless hole that won't comfortably hold any kind of jewelery. Thankfully, my piercing ignorance didn't get me into any trouble that day, but it did cure any of my doubts about piercing and left me with a strong desire to properly pierce something (anything!) on my body.

I came across BME as I was doing my piercing research and read hundreds of stories about every type of body piercing imaginable. Initially I chose the Prince Albert for my first proper piercing. It was completely new to me (maybe I live a sheltered life), but I loved the way it looked. The more I read about it, the more I was convinced that I wanted one, but in the end I decided against it because I wasn't ready to commit to a month without sex for healing. My second choice was the scrotal ladder. I guess I wanted to stay in the same vicinity of my body. Besides, a ladder can look a lot more imposing than a wimpy little ring through the cock anyway. I was going to build a ladder, and I was going to do it myself!
So I ordered all the necessary supplies online and went ahead with the first piercing, a "lorum" at the base of my cock. I'm not going to go into a lot of detail about the actual piercing. There are already plenty of other good self-piercing stories you can read to get the gory details. It took three hours to psych myself up, three minutes to actually do the piercing and a minute to put in the CBR (14 G). It was an amazing experience! I loved every second of it, and it wasn't really painful at all! The piercing seemed to heal up quickly and in three weeks I was ready for another rung on the ladder.
I did the next two piercings at the same time, one 14 G above and one 12 G below the first piercing. The top one was fine, but the lower one looked crooked. (It's surprisingly hard to poke a nice straight hole through your own body.) So I took it out and re-did it a week later. After a few more weeks I had three fully-healed rungs on my ladder. Or so I thought. Every one of the piercings proceeded to cycle through alternating periods of comfort and irritation. For a few weeks they'd be nice and healthy, and then one by one they'd turn red and painful again.
I decided that sitting at a desk every day was probably causing too much tugging on the rings and not allowing complete healing. To spread out the force on the piercing, I increased the size of my three rings gradually over several weeks until they were at 12, 10 and 8 G. I did it slowly enough that the stretching wasn't painful at all. At that point I was very happy with the look of my stubby little ladder and decided to give it a break for a while to concentrate on piercing my nipples. That's a separate story, but I'll mention as a point of comparison that the nipples were a MUCH more painful experience, but ultimately more rewarding. Nipple skin is much tougher than scrotal skin!
Eight months after starting my ladder and it was still painful. I tried changing the diameter of the rings. I tried going back to a smaller gauge. I tried wearing different clothing. Nothing would make the piercings settle down and behave. So I took out the most irritated rings (the top two) and gave up on the ladder. The bottom ring is still in today and doing pretty well. The other two holes have almost faded away to nothing. I'm not sure if it's the position or the size of the gauge that made the one ring succeed where the other two didn't.
I still think the scrotal ladder is one of the most impressive piercings and I'd encourage anyone to give it a try. But if you're considering it, I suggest you start with a single lorum and wait at least three or four months to make sure it doesn't get irritated by your daily activity. That being said, the lorum is a good first piercing because it's quick and minimally painful. Words of wisdom for the uninitiated: Piercing takes a lot of patience. Especially if you're going to get into something ambitious like a ladder, you should expect it to take a long time to complete.
