After having read a bunch of the experiences here on BME, a guiche seemed to be the next desirable step, as many report that the enhanced sensations are definitely worth the trouble.

After exchanging a few emails about placement and aftercare, I scheduled an appointment to swing by the shop and have the job done.
I prepped for the appointment by doing a little shaving down there, which I would later come to regret. Once I was clean and ready to go, I headed down to the studio. We selected the jewelry, he put it in the autoclave, and then had me go into the room and drop my shorts. My piercer then had me get on all fours as he looked for the proper location for the jewelry. After tugging at the skin behind my ball sac for a bit, he finally found the spot that kind of made me jump, and upon noticing that my dick was starting to get hard, decided that he had the right spot. He marked the location, and then gave me a mirror to see if I was satisfied with the placement. I was a little surprised to see that the spot he marked seemed further from my balls than I had imagined, but he said that I had enough skin for it and that the spot seemed to have the greatest sensitivity.
Okay—this is the part of the story where everyone says how big the needle is and the clamps being placed and the stuff about the canula being used, but to be quite honest—I'm a little shy of blood and stuff, so I told him not to tell me what he was using or what he was doing or any of that crap. When I had my PA done, I just looked at the ceiling the whole time, and I still almost fainted afterwards. My preference has become the following: just let me close my eyes, give me the count, do the deed, and slap a bandage or whatever on me when you're finished. So—if you're looking for all the details about this part—you're out of luck. Time to move on to the next writer. All I know is that I've got an 8-gauge cbb down there, and I don't really care all that much about what it took to get it there!
So, he counts to three, shoves the needle through (which seemed to take forever), I try not to scream like a girl, and he's got the jewelry inserted before I know it. The pain was worse than my PA or frenum, but lasted relatively briefly and was over. He cleaned me up, gave me the aftercare instructions again, had me acknowledge that I was still coherent, and sent me on my way. (Knowing how light-headed I get after a piercing, he had me pay beforehand to save me the trouble of trying to remember where my wallet was or having to sign the credit receipt.) Luckily, I wore loose boxers and baggy shorts to the appointment as he had directed me to do, so that made it easier to walk around afterwards. I discovered upon getting into my car that I had to sit just right, or else it would put pressure on the piercing and hurt like hell. If the DMV had any idea how poorly people drive after getting their crotch pierced, they'd never give us licenses.
I made it back to my place, ripped off my shorts, put down a couple of old towels, and just let the whole area down there hang free in the breeze. Luckily, I had a couple of days off from work, so I could just hang out at the house and let the thing heal. With my PA I had bled like a madman, so I wanted to make sure I could just stay home and bleed if I had to. While there was some blood the first four hours, after that, I had comparatively little bleeding. The hardest part about the whole thing was doing the soakings. With the frenum and PA, I could just whip my dick out of my pants, dip it in a cup, dry off and be on my way. With the guiche, I had to take off everything below the belt, position the shot glass, and hold it in place for what seemed like forever.
One of the things that was slightly different than my other two piercings was the healing time. My frenum and PA healed in no time flat, due to the fact that I soaked them often with sea salts and took things easy. The guiche seemed to stay rather sensitive (but not painful) for quite a while, and I continued to have the crusties or lymph or whatever it is build up for a good three to four weeks after the piercing. Shaving my crotch beforehand was a bad idea, because it started to itch like hell when it grew back in, complicating the healing process somewhat. Walking and sitting are activities that you have to get used to again, and you've always got to make sure that the area stays clean. I noticed that some pairs of pants were just too uncomfortable for the first few months afterwards, and I tend to still stick with the looser-fitting pants and shorts. It took a while before I felt comfortable enough to let a partner tug and play with the guiche while we're going at it, but I've got to admit that the extra sensation is definitely worth it—and having a go with it while I'm soloing isn't so bad either. Overall—the crotch piercing is a definite plus, despite the healing time.
