As long as I have had an awareness of the intricacies of an uncircumcised penis (when the prepuce finally fully detached from the glans), I have always wanted to modify my frenulum in one way or another. My first thoughts on the subject were about cutting it off, making the prepuce completely unencumbered in its movement. I attempted it, with many tools, from x-acto knives to nail clippers. Being so young, and not having the drive to overcome the body's reactions of self preservation, I was unsuccessful. It wasn't until later I realized that not all men have the frenulum intact - or have a foreskin at all. That gave it almost sentimental value. It was then I decided that I would only decorate it, to emphasize my "extra" parts.

I had been piercing professionally for a little over two years at the time, and had performed many genital piercings, and never noticed too much of an adverse initial reaction from frenum or foreskin piercings. This gave me enough extra confidence that I could do this to myself.
So it came to pass one night that I was bored, naked, and alone in my room. (a rather common occurrence during that period...) The idea came back to pierce my frenulum. I also decided that I would pierce my foreskin so as to keep it held back while the frenulum piercing healed.
I started by cleaning and disinfecting my work area. (the tray I would be working from and the chair I would be sitting in) I then set out clean dental bibs to protect the surfaces involved. I brought out a set of rounded mini Pennington forceps; three 12g needles; one 12g 5/16 steel cbr; and two 12g 3/8 steel cbrs. Then out came the alcohol and iodine (I didn't even hear about technicare until six months later... ) followed by the gauze pads, cotton applicators, toothpicks and gentian violet. Having my work area set up, I began prepping myself.
I began by shaving the area in question, and making sure to rinse it thoroughly with running water. (There are a few benefits to piercing yourself in the room next to the room in which you bathe) I dried off with paper towel, and washed my hands. I sat myself in my prepared chair, and another clean dental bib was brought out, a hole was cut in the middle, and my genitals pulled through. I was ready to begin the marking process. I cleaned the area with alcohol and gauze pads, and then let it dry.
Next, reference marks were placed: dorsal- midline, sulcus, and frenar band of the prepuce; ventral- midline and a transverse reference connecting the most proximal points of the corona. From these, the actual markings were placed- the foreskin piercings were at 45 degree angles, entrance at the sulcus, exit at the frenar band; the frenulum piercing was approximately 3/32" proximal from the distal end of the frenulum, at an angle parallel to the coronal reference, entrance and exit 1/32" ventral from the sulcus.
Then I swabbed the area with the iodine, and let that dry. Using only enough pressure to hold the skin firmly, the forceps were put in place for the frenulum piercing. (normally I wouldn't have used the clamps on a frenulum, but it makes it easier when piercing oneself) I lubed the needle with surgilube, and got it into position. A deep breath in... and by the time the needle touched the entrance side, it was out the other. Only mildly uncomfortable, with that slightly sick feeling in the stomach brought on genital injury. Forceps were placed for the left foreskin, the needle lubed and positioned, a deep breath in... as I exhaled, I pushed the needle through, it took about three seconds, not long in comparison to some things, but in piercing, that is quite a while, especially for such thin skin. The time for number three had come. It went exactly as the prior, and the pain from both was about the same as from earlobe piercing. For the time being.
I bandaged my new adornments, played video games for a while, and went to bed.
I of course was awakened in the middle of the night by the pain caused by the combination of fresh piercings and an erection, followed by the frustrating wait for things to return to normal so I could sleep again. This was a normal occurrence for the next few days, and during the days, it was uneventful for the most part. I cleaned them once a day with antibacterial soap, and for the most part let them heal on their own.
After about a week, my pants constantly rubbing on the foreskin rings became more of a painful sensation than a discomfort. There was no discharge, and the piercings didn't seem irritated, so I removed the jewelry and flushed the piercings with saline.
At this point, the piercings had already epitheliated to the point that they could hold the foreskin back without jewelry. I didn't try to bring the foreskin over the glans yet, as I didn't feel like causing any more damage that day. About four days later, I decided to pull the foreskin back down. With a slight burning, tearing sensation, the skin was returned to its natural state.
I continued to keep the frenulum piercing clean with the soap, but after another week, the jewelry started to pinch the skin involved, so I took a closer look at the situation. The fistula had enlarged to about 8g. I took out the ring in hopes of it shrinking a bit, because at the time I had had enough hassle with these piercings, and I figured a 7/16 8g cbr (the smallest I had the ability to make at the time) would be pretty uncomfortable under my foreskin, seeing as it added 1/8" with the inner diameter, and almost another 1/8" with the larger gauge.
A month passed, and the hole was the same size. I got the idea to make an 8g 1 1/4" cbr, one that would hug underneath the corona of the glans, taking up only 1/8" around most of the foreskin, and 3/8" for the bead, which actually ended up more comfortable. It kept the full weight of the jewelry off the thin skin of the frenulum, and barely took up any space at all.
Overall, I am very happy with my frenulum piercing experience. I wouldn't recommend the average person to do this to himself, but gutting these piercings is just a minor hassle if things are done properly, and you have patience.
