Well I had a true first yesterday (Friday) evening. I did a double foreskin piercing, one on each side of the head. And like the apadravya last week, he just walked in and wanted to go for it.
I was a little nervous about this piercing because I'd never seen an uncircumcised penis up close, let alone manipulated one. I was at a disadvantage, and was honest with him about it. We talked for a while and he answered my sometimes very ignorant questions. I am still amazed at how different the glans of the penis looks when it hasn't been subject to years of rubbing against cotton underwear. It was so fragile and sensitive looking that I was afraid I was going to hurt him while cleaning him up. I never realized how much skin they remove when circumcising. It made me wonder what sensations I might be missing.
Now, I know how the finished product is supposed to look, and I knew that he wanted to someday wear just one ring through both piercings as a form of infibulation, but the actual placement of the piercings was somewhat a mystery given that the skin is so elastic. He explained that the skin at the tip of the penis pretty much stays where it is when he gets an erection, so placement of the rings became a little easier. We started out (after cleaning) by placing dots on the outside of the foreskin where he wanted the rings. We were using 14ga 3/8" niobium rings which fit his anatomy well. I then clamped the foreskin using modified Penningtons and made a mark on the inside of the foreskin. After removing the clamp we then pulled back on the foreskin to the point that he said the skin usually stretches when erect, and looked to see where the dots ended up. With a little experimentation we felt confidant that everything was perfect. Reclamping with just the barest pressure I pierced one side and finished the jewelry. Moving to the other side, I pierced, but when I went to insert the jewelry the skin on the exit side wouldn't pop over the jewelry and there was a tense moment as I had to press the skin down over the ring. He said that he didn't feel it, but I was sweating bullets. I didn't want to lose the pierce. Next time I will be more firm in pressing against the exit side of the foreskin when inserting the jewelry. The foreskin is so elastic that unless your fingers are right against the needle it can just stretch out.
Mission accomplished. He was happy, I was relieved. Then he turned green and had to recline in the chair with a cold towel on his head. I use a hydraulic dental chair and he said he always passes out at the dentist's office. Same thing when he gives blood. (Oh, thanks for telling me now!) I was a little behind schedule after that, but it was worth the experience.
It seemed funny that the natural state of the penis was such a mystery to me. I've never had the opportunity to watch someone else pierce a foreskin, so we were operating on intuition and common sense. The vagina and circumcised penis present no problems because I have had ample opportunity to explore them. My wife was the pin-cushion for my first solo attempts at all the female (non-clitoral) genital piercings, which was perfect because I could take all the time I needed. And I performed most of the male genital piercings on myself before ever trying them on a paying customer. (We took most of them out right after getting pictures.) Then came the stream of friends willing to be discount guinea pigs. Now I feel that I have crossed the final frontier, as the foreskin was the only mainstream piercing I had yet to accomplish. As rare as foreskins seem to be, I may never master that piercing, but everything turned out fine and looked just the way we expected. Just lucky I guess.
Any suggestions on how to better perform foreskin piercings? I would appreciate any info thrown my way.

My client tells me that he is healing very well and experienced much less discomfort than he expected. He also told me that it is a bad idea to try to masturbate the day after the piercing. He said it hurt like crazy when the rings started flopping around. He also let me know that even though the rings hang perfectly when he is not erect, during an erection the foreskin rotates about ten degrees around the head so that when he eventually wears a single ring it will be slightly off center. He is very happy with the outcome. Me too.
