After having my wonderful PA for about a year, it had gone from a 10g to almost a 4g. I had become very worried about causing my own meatotomy, without even wanting one. I talked with Scott and we decided that converting my PA to an Apadravya would be the best route to keeping metal in my penis.

1. Put a receiving tube into the PA and piercing through the top of the glans penis.
2. Dermal punch straight through to the urethra.
3. Scalpel through the top of the glans penis towards the PA hole and then stretch.
After some more talking, piercing was not the way I would get the Apadravya and it was decided that dermal punching would do it. I got excited because I was going to get to eat the punch from my penis. Sure, it seems gross, but waste not, want not.
A couple of weeks went by until the jewelry I had ordered arrived. It was a 6g, 1 and 1/4 inch barbell with a dented bead that had a 8g ring attached. Once it came in, we got down to business. There was more discussion about the punching and a decision was made that it would probably not be the best route. Therefore, plans were changed and scalpel became the word of the day. First things first, I got EMLAed and went out for an hour and a half while it soaked in. I even had EMLA put into the urethra. That was insane. If you have never done sounding, then seeing a Q-tip covered in EMLA, acting almost like a catheter, disappearing into your penis is a little disturbing. It stung about as much as when you get soap into your urethra, if not less than that.
When I returned, everything got set up. Sitting, somewhat ominously, on the tray was a #11 scalpel and my 1 and 1/4 inch, six gauge barbell and associated taper. Seeing the scalpel did not scare me too much. I had seen tongue bifurcations and almost had one used when I got my pearling done. I would even go so far to say that seeing the scalpel got me an extra bit excited about getting my Apadravya. The necessary cleaning supplies were also seated comfortably on the tray. I lay down on the massage table and Scott cleansed the area, and tried to mark my penis. My penis was not cooperating, and it was quite difficult to get the mark exactly where I wanted it, but when it was there, it was there, and all was good.
Meanwhile, Dan, my tech support, was trying to operate Scott's digital camera. There would have been a movie, but the camera did not want to cooperate.
After placing the forceps uncomfortably around my penis, which caused it to look as if someone had stepped on it, and taking the customary deep breath and exhaling, Scott quickly went to work. He accurately and effortlessly pushed the scalpel through to the urethra and then with the skill of a professional ninja, inserted the taper followed by the 6g barbell through to the PA hole without missing a beat, or the PA hole.
When everything was said and done, Scott informed me that I should not be surprised if I fill up three gloves with blood. This certainly scared me (of course he had told me before about the blood, but it never actually hit until after I got this done). Well, for the first day or so, I hardly covered a full gauze pad in blood. Over the course of the week, I had an exponential drop in the amount of blood escaping from my penis. In fact, the only time I remember seeing a mass amount of blood was after the barbell had been put in. Peeing only burned for the first 4-5 days or so. Then set in the crusties. At first, just one big lump of crust would flake off whenever I took my shower and sea salt soaks. After three days, the crusties have all together disappeared. Unfortunately, they reappeared in force and continually cause the top ball to get attached to my penis. As of now, the top of the glans has completely healed and I am peeing out of three holes. I have tried m asturbation once or twice, but once some blood starts leaking out, it's all over. So, I guess that I still have to wait on the inside to heal. Another problem with healing was having that 8g ring on the barbell. Every once in a while, my movements would put torque on the ring and turn the barbell. This was quite painful and I took off the ring and put a normal bead on. I'm so glad that I had EMLA for this. Not only did I feel almost no pain, but also there was little swelling and had very little blood loss. Satin soap was used for cleansing and healing. This is a great product. Antimicrobial and it promotes healing!
