I created a stirrup for each nipple with bicycle spokes, a torch, 2 pairs of needle-nose, a hammer, and a piece of 3/4 inch (2 cm) square steel rod from the hardware store to use as an anvil. For measuring the spokes I prototyped the stirrups with easily-bent wire cut from a metal coat hanger.
(A stirrup is a U-shaped piece of metal with holes at the top for a barbell to slip through.)
I bought an ordinary "choke-chain" dog collar from Wall-mart or the local dollar store, about 16 or 18 or 20 inches (about 30-50 cm).
The dog collar chain was of the ordinary sort. A length of close link chain with a welded 1-inch ring (2-3 cm) at each end and a welded choke ring, same size, in the middle of the chain.
I used a power tool and took off the 4th link of close chain from one large ring on the dog chain. By holding the link with a pair of needle-nose in one hand, and the power tool in the other hand, I could take off the 4th link. (Note: after cutting, the chain will be HOT!)
If I wanted to give her more weight on her nipple I could have taken off the 5th link. But then I was sure the larger rings at the end of the chain would dangle from under her top and be exposed, even with the tee-shirt tops she normally wears.
The human female usually wears a top, covering her boobs, in public. I could not have even started this project if the chains were going to be sticking out from under her top when she was doing ordinary things, like going to the store or the bank.
I took the same tool and took the 4th close-chain link from the opposite end. Again I could have left the 4th link and taken the 5th but I value balance in all things. If I had taken the 4th link from one side and the 5th from the other, I would have not liked it because the chains would not be identical. Besides the sticking-out problem, of course.
When I was done taking off the 4th link from each side, this left me with two pieces of slag (the ex-links) plus a free welded ring, a free piece of close-link chain, and two pieces of chain each with three close links and each with a big welded ring on the end. Then I used the hammer, the torch, my anvil and a grinder to fit the stirrups over rods of the proper gauge for each nip, and also made sure the open chain links would fit over the loops at the top of each stirrup. My plan was to use a taper pin to widen the link if I needed to, but it wasn't a problem.
I did not want to have it wrong when I went to attach the chains. So I went to the hardware store and I bought ordinary round steel stock in 6-gauge and 4-gauge so I could hammer the stirrup-loops into the proper dimensions, flattened and smoothed, before I attached them.
After making very sure that the stirrups were going to fit, Lefty fitting onto a spare 6-gauge barbell and Righty fitting onto a 4-gauge piece of steel stock, I attached the chains to my wife's nipples, on her current barbells, using the stirrups.
I invited her to remove her top and she complied. Then I suggested that she shut her eyes and place the backs of her hands on the top of her bare ass cheeks. She complied. I took the needle-nose pliars and removed 3 of her 4 beads. The 4th would not come off. I used a lubricant and a taper pin to force out her 4-gauge barbell so I could put on her stirrup. I put the stirrup onto the taper pin then replaced the original 4-gauge barbell, with one bead still on it, through the openings in her right stirrup.
My wife has a different size bar in each nipple. I do not like this but it is a fact of life, which I can not do anything about.
She wore her nipple chains for five weeks. I could hear the jingle of her chain everywhere in the house as she moved from room to room, and the sound alone told me where she was. It was a nice sound (-:
I could attach her chains to pretty much anything I wanted. Because when she was chained too far away from the tool cabinet to get to the needle-nose pliars she knew she needed for taking off her chains, when I chained her short she knew she was stuck. I left her a bucket sometime to pee in, if she was chained short and I knew I would be gone for a while.
I would sometimes make a great show of removing the needle-nose from the cabinet, and show her that her chain was long enough to reach the cabinet but no tools, and then I would leave her nipple-chained to some object for several hours.
Then there came a day when she needed to wear a really tight shirt and there was no way she could conceal her chains. Of course she could have worn a bra but she'd tried that already (she has one bra, only one, and only for rare use). The bra would totally stop the jingling sounds but even with her bra on sometimes a bit of her chain would find its way out. And this was --not-- a top that went well with a bra. So she decided then to remove her chains.
On her smaller, 6-gauge barbell, she managed to remove both her stirrup and her chain.
On her larger, 4-gauge nipple, she could remove her chain but could not remove her stirrup because one of the beads is bound, and can not be removed.

The moral of the story: If You are going to give your wife (girlfriend, lover, sex-slave, whatever) both stirrups and chains, with permission to remove her chains at her wish, you must make sure one bead of either end of her barbell is bound and can not be removed, or else when she takes off her chains she will also take off her stirrups.

Because we lost the stirrup that belonged to Lefty, Lefty is today without a stirrup, and Righty continues to happily wear her stirrup, even under a very tight summer top. This is ten weeks later. Now to keep her chained up around the house I just attach the house-chain directly to her stirrup. It's not balanced that way but it does the job (-:

CAUTION: Hanging long-term weight on a smaller-gauge barbell, or a ring either for that matter, will likely result in "cheese-cutter effect" with the result that the piercing will tear. At least once a week, she would get a chain snagged on something around the house and it would get a pretty sharp tug! Don't try this with smaller-gauge jewelery.
